Patient safety incident reporting is one of the steps taken needed to improve patient safety. Reporting can provide a broad picture and how it happened. This be used as basic data for policy making programs in hospitals. The subjects this study were health workers. research method uses systematic review, from various sources that has been done previously. databases are Science Direct, Scopus SpringerLink, journals published January 2011 December 2021 which then extracted using PRISMA 2009 flowcharts. So final results 11 reviewed. resulted incidents influenced by individual factors, group/unit factors organizational factors. conclusion improved evaluating improving
